Dealing with bad cramps?

I got this the day before yesterday, and today the pain is absolutely killing me. I took some Neoofen Forte, but it didn't do a thing... Now I'm sitting here at work wondering if I should just head home. If I need to take two days off, should I be calling my primary care doctor or my gynecologist for a medical note?
