How to switch OB-GYNs: What's the process?

I’m not entirely sure if I’m posting this in the right spot, so my apologies if it needs to be moved elsewhere! :-)
I really messed up. Over a year ago, I decided to switch gynecologists. I went to see the doctor I actually wanted, gathered all my medical records, and took them over to the local clinic on Jukić to get everything certified. I was supposed to hand one of those copies to my new doctor, but because of a sudden move to a different city, I never got around to it. In the middle of all that moving chaos, I managed to lose the papers entirely. So, what’s the play here? Can I just go to my new doctor and explain the situation to them and their nurse, or am I forced to head back to the clinic on Jukić to request a new certification from Medicare??
