Dealing with hemorrhoids – any advice?

My boyfriend is dealing with some pretty significant hemorrhoid issues. He was actually hospitalized for a bit and had to take several months off work while on medication. During that whole time, any kind of sexual activity was strictly off the table due to the pain. (Just for context, I am his girlfriend.)
What I’m wondering is whether it’s realistic for a doctor to finish a final checkup and tell him that he’s done with his medical treatment—meaning things should stay stable for a while as long as he sticks to a healthy diet and lifestyle—but then still forbid any sexual activity for another three weeks. To be honest, that feels like a lot to me.
I understand that you can't do anything when you're in pain, but what is the actual purpose of those extra three weeks of abstinence if the pain is gone and the medication is finished?
