Does anyone have a recommendation for a top-tier surgeon and a premier method to resolve hemorrhoids once and for all? Back in 2012, I underwent a Hal-RaR procedure. At the time, I had a feeling the surgery wasn't performed quite right, but I kept telling myself everything would turn out fine. It started with painless bleeding accompanied by anal pressure just two or three months after the operation, and now I find myself back where I started, hovering between the second and third stages. My doctor previously mentioned that we could repeat the surgery, but I kept putting it off; however, I can no longer live like this because it is clear things aren't improving. I am also hesitant about whether I should return to the same doctor for a follow-up procedure, because if the first attempt wasn't successful, I question why a second one would be. That initial surgery was performed at a private clinic in Washington, D.C.
Currently, I am undergoing rubber band ligation at a different clinic. I have already had three sessions done, and there hasn't been much improvement over how I felt before. Specifically, the left side, which has been subjected to ligation twice, still prolapses during bowel movements, though once I push them back in, they stay put for a while. On the other hand, the right side, which was only ligated once, prolapses again after I walk or stand for about ten minutes. This clinic doesn't perform Hal-RaR; instead, they perform the Morgan-Milligan procedure.

I honestly don't know anymore. I am feeling incredibly frustrated because, for the first time, I actually believed that my surgery would provide the relief I needed, but now I am starting to have serious doubts about that being the case.

It is now the second day, and while the hemorrhoid has increased in size, the pain has actually subsided somewhat. I managed to get some sleep last night, which was a significant improvement over the night before. I suspect this might be a thrombosed hemorrhoid, though I cannot say for certain. My plan is to wait until Monday to see if it finally begins to recede, but I am honestly uncertain about how that will play out given that it is currently as large as a walnut. What is the typical protocol if it is indeed a thrombosis? From what I have gathered, this can be resolved at a clinic through a minor surgical procedure that is supposedly quite painless. However, I am curious if anyone here has experience regarding how long it takes for a mass of this magnitude to shrink. It truly is enormous; I have never experienced anything quite like this, even during my worst flare-ups prior to my previous surgery.
I am considering waiting once again to see if I can manage the situation through dietary changes to keep things under control. I am conflicted. On one hand, I could commit to a long-term struggle with dietary adjustments, or on the other, I could simply opt for surgery tomorrow.
I am genuinely wondering how much time it will take to subside because this specific one is massive. Looking at it, I have this sensation that it may never retract because of how large and firm it feels.

They’re back again in the absolute worst possible state. To give you the short version, I underwent surgery at Leptir Clinic about two and a half years ago using the Hal-RAR method. For a while, it seemed to be holding steady, but about six months ago, things started shifting again. I went back for a check-up and spent three months on Daflon, followed by ten days of Medazol and some cream. Things stabilized briefly, but they didn't actually go away. My routine has been to just wash up after using the restroom, push them back in, and try to move on with my life. But yesterday, they flared up so aggressively that it felt completely out of control, and I simply cannot get them back in. I haven't slept a wink all night or during the day; it has been an absolute disaster. The inflammation is so intense that the pain is driving me to the brink of insanity. I picked up some Pilex for the first time to see if it helps, alongside various creams and other pharmacy supplies, but honestly, it's just pure drama. I paid a massive amount of money for this surgery, and in the end, it hasn't helped me one bit. So where does that leave me? Do I really have to go through with another surgery? When I saw the doctor six months ago, he mentioned we could repeat the procedure, but that means paying a fortune all over again, enduring those insane post-op pains, and still having zero guarantee that everything will turn out fine. This isn't even a matter of logic anymore. How often do hemorrhoids recur after the Hal-RAR method? They claim a success rate of up to 95%. Am I just incredibly unlucky, or did the doctor fail to perform the procedure correctly?

Michael Diaz4, you seem to know a lot about Hal-RAR, or perhaps someone else might know more.
What am I left with here? Should I just live with this and suffer, or undergo surgery again? I don't want either option; I just want to be cured.🙂🙂🙂🙂🙂

Ten days ago, I underwent surgery using the HAL-RAR method... moving from the second to the third stage...
I went home the day after the procedure. Immediately following the surgery and my return home, I dealt with unbearable pressure and pain, though everything has stabilized since then.
When I have bowel movements, I occasionally experience quite intense burning.
Is this simply because my skin is sensitive and slightly damaged following the surgery, or is it something else?
Today, after using the restroom, I feel a pulsing sensation in my rectal area, much like what I experienced before the operation, and it feels as though my hemorrhoids are still "working" or pulsing internally just as they did before.
It is somewhat frightening to me... is this considered normal?
Additionally, after my last bowel movement, I felt a small protrusion near the anus that wasn't there a few days ago. Looking in a mirror, it appears as though a small lump has emerged; the area simply looks different than it did two days ago.
What have your postoperative experiences been like for those of you who underwent the HAL-RAR method?
Did anything similar happen to you? Did your hemorrhoids truly disappear for good after the surgery?

They vanished forever. Though, I will note that the physical configuration of the area has changed...

I originally wrote the post above on April 29, 2012. It has now been exactly two years since my surgery. My colleague Michael Diaz4 responded to me stating that they were gone for good, but I fear that isn't the case. Last summer, during the fall and winter, my hemorrhoids used to bleed sporadically. While they aren't bleeding now, I frequently notice traces of blood on the stool and the toilet paper. Furthermore, my left hemorrhoid used to retract immediately after a bowel movement, but now it takes much longer to go back in, and it is no longer as small as it once was—though it is about half the size it was prior to the operation. The pain isn't nearly as excruciating as it was before the surgery, but there is still discomfort after a bowel movement, even if it no longer reaches that unbearable level. Worst of all, it feels as though they are returning, and the mere thought of undergoing another surgery is terrifying.
For the last two months, I have frequently felt a sense of pressure in my rectum, as if I need to have a bowel movement, yet when I go, nothing happens other than passing some white discharge. Additionally, my stools have been quite thin lately, which is very unsettling. The most difficult part is that this rectal pressure and the post-defecation pain have become increasingly frequent.
The constant pressure and pain are taking a significant toll on my mental well-being. I am at a loss as to what to do next.
Should I continue taking Detralex, even though my doctor told me it wouldn't be necessary anymore?
Should I consult a doctor, whether it be my current one or someone else?
I've considered hemorrhoidal ligation where they eventually fall off; what are people's experiences with that?
Should I attempt any natural remedies, even if I don't know which ones might actually work?
My diet is generally quite solid, avoiding spicy or heavily seasoned foods.
Do I need to increase my intake of fruit and fiber?
Am I contributing to the problem by spending too much time sitting on the toilet or straining when necessary?

At this point, I don't know anything other than the fact that I feel like I might lose my mind if I have to go through this all over again. This rectal pressure is debilitating and uncomfortable, and it is seriously affecting my psyche. On top of that, it is becoming more frequent.

Michael, or anyone else, do you have any good advice or suggestions?

I just finished in the bathroom, rushing through it so quickly that the hemorrhoids didn't even have time to emerge. The stool was thin; there was no pain, but I can still feel them, along with a slight sense of pressure.

I went home the very next day after the procedure. Shortly after returning home, I dealt with terrible pain and high blood pressure, but since then, everything has seemed fine.
However, when I have a bowel movement, I sometimes experience quite intense burning.
I am wondering if this is simply because the skin in that area is sensitive and slightly damaged following the surgery, or if there is something else going on.
Today, after using the bathroom, I feel a pulsing sensation in my rectal area that feels remarkably similar to what I experienced before the operation; it feels as though the hemorrhoids inside are still "working" and pulsating just like they used to.
This is somewhat frightening to me. Is this a normal part of the recovery process?
