Dear Sir or Madam,

Could you please provide some clarification regarding these lab results?

Thank you.

In the bone marrow sample, cells the size of lymphocytes (14%) and monocytes are present.
The granulocyte count is at 8%, while the granulopoeia stands at 78%. Based on the mononuclear cell analysis (which includes lymphocytes and monocytes)...
The findings indicate that T+NK cells make up 38%, while B-cells account for 10%. Additionally, monocyte phenotype cells represent 19% of the total.
Immature myeloid cells at 17%, which includes 9.5% of cells exhibiting a myeloblast phenotype.
Out of the total number of stations in the sample, the proportion of T+NK stations stands at 8%, while B-type stations account for 2%, and the station type...
Monocytes at 4%, immature myeloid cells at 3.8%, including 2% of cells with that specific phenotype.
Myeloblast, along with findings showing granulopoiesis at 78%. The results show no pathological signs.
Mononuclear cells of the bone marrow.

It is now time to rule out any potentially malignant causes for this constellation of findings you have presented. Let us hope the test results do not lead in that direction. Not everyone has to wait two months for all results to arrive. Cytology results are typically ready very quickly, and immunophenotyping results usually follow shortly thereafter. While FISH and karyotype testing may take a bit longer, you might want to inquire about the cytology (often available one or two days after the biopsy) and the immunophenotyping (usually within a week, though timelines may vary due to seasonal staffing changes or holidays, but certainly not two months). Best of luck.

Thank you for your response.

CYTOLOGY RESULTS:

Myelogram: blasts 2%, promyelocytes 4%, myelocytes 9%, metamyelocytes 4%, non-segmented granulocytes 17%, segmented granulocytes 19%, lymphocytes 14%, monocytes 1%, erythroblasts 30%

DIFKS segmented granulocytes 42%, lymphocytes 47%, monocytes 8%, eosinophils 3%, E: normocytic, normochromic. Trace amount present.

In the bone marrow aspirate smears, moderately abundant hematopoietic tissue was found, where the ratio of white to red lineages is approximately 2.3:1 in favor of the white lineage. Erythropoiesis is mature and predominantly normoblastic. Granulopoiesis is represented by mature and transitional forms. Thrombopoiesis is represented by mature megakaryocyte forms.

If you could please clarify the findings above, and explain what will be determined via tissue phenotyping, FISH, and karyotype.

Thank you.

Best regards,

Based on what I am seeing, you are experiencing lymphopenia, which requires further investigation.

The primary question is, what does your blood work typically look like? (I assume you have had blood counts performed at various times throughout your life.) In other words, is this deviation in your leukocyte count a new finding since January 2022, or do you have records indicating that your leukocytes have always sat at the lower end of the normal range? Have you been ill within the last 6 months? Is it possible you dealt with an infection, a fever, or generally felt unwell during that period? Have you undergone any medical examinations relatively recently? Do you engage in high-risk sexual activity? Are you currently taking any medications? Do you consume alcohol? What is your body weight, and has it changed over the past 6 months? Have you experienced any severe infections in the past? Any fevers, night sweats, rashes, or skin changes?

My blood work from June and July remains unchanged, with a continuing decline in both leukocytes and lymphocytes.
Additional tests including HIV, Hib, minerals, folic acid, EF serum protein, and B12 all came back normal.
I am currently waiting on results for tests already performed: a bone marrow biopsy and analysis, immunophenotyping of the bone marrow, FISH, and a karyotype. They told me at the Mayo Clinic that I would be waiting about 2 months.

I am not sure what to expect.

Thank you.
Dear all,

I would appreciate your insights regarding my recent lab work. I am a 36-year-old male, otherwise healthy, fully vaccinated (two doses), and have never contracted COVID-19. I had my first blood panel performed in January, and the results were as follows:

WBC 2.7
NEUTROPHIL GR 1.60
MONOCYTES 0.30

These findings were repeated in the second and third months, showing similar consistency.

April 14, 2022
CYTOLOGY REPORT RESULTS:

eosinophilic granulocytes 1%
segmented neutrophil granulocytes 54%
lymphocytes 31%
reactive lymphocytes 10%
monocytes 4%

RBC: mild anisopoikilocytosis, normochromia
WBC: normal morphology
PLT: present, normal morphology

The recommendation provided was a full hematological evaluation.
