Dealing with hemorrhoids – any advice?

If an external hemorrhoid pops up and hurts like hell for about 4 or 5 days—but there's absolutely no bleeding involved—and then suddenly the pain just vanishes, leaving behind this tiny little bump about the size of a pea that’s shrunk by maybe half... is it actually okay to just leave it alone?
Basically, if it stays like that permanently—meaning it doesn't hurt anymore and there's no blood—should I just stick to good hygiene and keep using my creams, or am I supposed to go racing to the doctor to demand they just cut it out surgically?
