Dealing with hemorrhoids – any advice?

Hey,
This is my first time dealing with this kind of mess. I feel like I checked every single box for developing hemorrhoids: genetics, spending way too much time on my bike, sitting all day at work, and then throwing in a few days of bad eating, drinking, and smoking...
It’s an external type and basically popped out overnight. No bleeding yet, but it hurts like hell, especially when I'm using the bathroom.
I’ve already started using cold compresses and taking lukewarm or cool showers.
Can anyone recommend specific meds or creams? I see so many different things mentioned on here—Pilex, Hemopropin, etc.
Also, should I go get a medical exam? I'm worried if it's something like a thrombosis where surgery might be the only fix.
