Dealing with hemorrhoids – any advice?

I’m heading to the doctor in a few days because I’ve been seeing blood in my stool for quite a while now... Back when my diet was terrible, it was just on the toilet paper, but now it's actually in the stool. It hurt pretty bad today when I went to the bathroom, and there were streaks of blood visible... Honestly, I'm terrified it might be stage 4 and I'll end up needing surgery..
Can anyone tell me what an exam for hemorrhoids is actually like? And does it hurt?
Basically, whenever I have a harder bowel movement, the blood shows up...
But I don't get why it's showing up in the stool now instead of just on the paper??
Before I started hitting the gym and eating better, I used to see blood on the paper all the time... I haven't had any issues with blood on the paper for about two years, but now it's appearing in the stool itself, which makes no sense to me
