Looking for a specialist...

I’ve been dealing with this issue for a while now—the skin just won't retract. I know the standard route is to see my primary care doctor to get a referral to a urologist, who would then decide if I need a circumcision or something similar.
The problem is, I can't seem to get an appointment with her. I'm wondering if anyone knows a urologist in Washington, D.C. who accepts Medicare. I'd like to establish care there and, if possible, have the procedure done during the same visit.
Any recommendations?!!!!!!
I've already sent out several emails, but I haven't found anyone who takes Medicare.
Otherwise, I might just pay for a consultation and go to a hospital to get it handled.
I looked into private specialists, but the costs are $1333 quite high. If I can't find a way to do this through insurance, that's my backup plan, even though money is tight right now.
Also, does anyone know how you transfer your medical records to a different doctor?
