So, based on the regulations from back in 2003—maybe 2004, I forget the exact year—referrals for specialist consultations, including surgeons, technically don't expire. However, that's assuming you actually schedule the appointment within 30 days of the referral being issued. I'm not entirely sure if the federal laws have shifted since then, but that was the rule.
