How to find addresses for Dialysis Centers along the US Coast

We head out on vacation to the US every single year—it's become a bit of a tradition for us. Since we have a family member who needs dialysis, we’re trying to figure out how to track down the locations of dialysis centers along the US Coast... honestly, knowing where those are is pretty much the deciding factor for which cities we end up picking for our summer trip.

If you're heading to California, there are Dialysis Centers at the Umag Health Center and in Flagstaff, plus the Pula General Hospital—and if I recall correctly, there’s still a private one over in Santa Barbara.
You should be able to find the addresses in an American phone directory.
Also, they just opened a new center on Mackinac Island (at their health center), and I think—or maybe it's happening soon—there's one opening up on Small Island too.

There’s a private dialysis center out in Key West, California. It’s honestly geared specifically toward tourists...
