Hey.

After bouncing between several urologists and getting nothing but wrong diagnoses—like being told it was just prostatitis—only to eventually be told everything was perfectly fine, I finally got a real answer: suspected asymptomatic Urethritis.
It all started three months ago. I'm dealing with constant urination, sudden and uncomfortable urges (averaging about 20 times a day), tension in my penis, and occasional weird irritations. It feels swollen or inflamed, though not constantly intense. Then there’s this numbness that’s killed my sex life entirely, plus some erectile dysfunction issues...
The frequent bathroom trips aren't even the worst part; it's that irritating pressure on my bladder. As soon as I feel even a tiny urge, I have to rush to the restroom just to squeeze out a few drops.
I'm currently on day seven of Doxycycline, but so far, zero change. My doctor also put me on Urotri for the next two months. I have a follow-up next week. I'm seriously considering more invasive tests, maybe a urethroscopy or cystoscopy (not entirely sure which is which).

Has anyone dealt with anything similar? What did your doctors actually find, what tests did you undergo, what antibiotics worked, and what was your experience like?

What kind of testing was done? Scans? Swabs?

Pelvic ultrasound covering the bladder, testes, and prostate; transrectal ultrasound (TRUS); prostate exam. Urethral swab and ejaculate culture (normal flora at 10*3 CFU/ml); Mycoplasma/Ureaplasma swabs and ejaculate (no Ureaplasma spp./Mycoplasma hominis isolated); Chlamydia trachomatis swabs and ejaculate were negative.
Urine culture: < 10*3 microorganisms per 1 ml.
Urinalysis sediment: WBC 3-4 (ref. 0-2), non-hyaline casts 1-2 (ref. 0, note: granular casts), trace bacteria, heavy mucus...
Bloodwork:
MPV 6.5 L (ref. 6.8 - 10.4)
The second urinalysis showed plenty of bacteria, more mucus, and elevated WBCs.
By the third urinalysis, there were still some leukocytes, RBCs, and cells with 10-15 WBCs. That’s when they put me on Doxycycline.

Latest exam (TRUS) shows the prostate looks symmetrical, no clear hypoechoic areas.
Earlier exams (ultrasound and manual exam) showed an inhomogeneous, softer prostate that was slightly tender. Radiology noted some hypoechoic areas in the prostate.

Forgot to mention I also had tumor markers checked. The report shows S (Associated Press) at 2.0, with a ref interval div>
