If you physically cannot pull the skin back over the head, you're dealing with phimosis. If it's only an issue during an erection, then it's considered partial.

Phimosis needs to be addressed for hygiene reasons—otherwise, you risk inflammation of the glans and other complications—and to prevent tearing of the skin or the frenulum during sex.

Treatment depends on how severe it is. My advice would be to talk to a doctor first and see if you can stretch the skin using steroid creams. Otherwise, surgery is an option, usually done as an outpatient procedure under local anesthesia. It's nothing to fear; it's quick, and you go straight home afterward.
