That’s incredibly rare, usually only happening in cases of severe trauma.
For instance, the heart can actually "bulge" out—essentially a cardiac hernia—during its contractions, which can mess with its rhythm depending on how big or where the rupture is. Then, if a blood vessel actually bursts, you could end up with bleeding in the chest cavity. Or, a significant amount of blood might get trapped between the heart and the pericardium, which crowds the space the heart needs to contract (since the pericardium doesn't stretch). All of this can lead to cardiac arrest.
