The usernames that ended up looking like what you described were either:
a) accidentally wiped out when the database crashed a few years back; or
b) banned/mass-purged.
If I recall correctly, that's how it went down...

Yeah, they were deleted by mistake and never restored, which is why things are the way they are. I don't know much more than that; maybe one of the admins would have a better idea.😍

If I’m remembering correctly, there was once a situation where someone hit 100 cards and just got wiped out, leaving them with a "patient" status... though I can't be totally sure, it might just be an old Reddit rumor 😬

Reddit never actually wipes its users—unless you register and then don't post a single thing for six months straight. But even if someone’s been totally MIA since 2003, they just stay in the database. I think the "patients" were just the users who got purged during the system crash...

That "Patient" label probably just pops up automatically whenever someone gets scrubbed from the database, based on how the ACP is configured. It’s similar to when an Admin assigns someone as a moderator through their settings—it defaults to "Moderator," even if they decide to change their display name later on...

I'm pretty sure that only happens if the user doesn't have their own custom tag set. In my case, it didn't change at all when they first set up my account. 🤷

It was similar on an old vBulletin board where I served as an Admin. By default, everyone showed up as a Registered User, but once I promoted someone to a moderator role, their tag would switch to Moderator. However, what you mentioned is likely the case—if there isn't a specific custom title assigned, the system might only trigger the change once the default "Registered User" status is overwritten.

Logically speaking, moving a user from one group to another should automatically trigger a title change. For instance, shifting from Registered users to Administrators, or from Registered users to Moderators, or even to Banned users... it just follows that flow.

The user groups on Reddit are:
-Registered users (default)
-Administrators
-Supermoderators
-Moderators
-Banned users

Not quite.
It didn't show "moderator" for me either.
The mods who actually have that tag in their signatures just typed it in themselves.
