Extremely painful lumps on my feet/toes

Basically, about ten months ago, I started noticing that what I always dismissed as just a pimple or two on my leg (sometimes on my glutes, mostly on the lower leg) had spiraled out of control and taken over a huge chunk of the area. To be honest, I’m not even sure if we’re talking about pimples, acne, or something else entirely, because I don't deal with anything remotely like this on my face.

Alright, I'm posting a few photos here, but since they are honestly pretty gruesome to look at, I wouldn't recommend viewing them unless you have a strong stomach:

I should add that these photos actually look mild compared to how inflamed everything was just two months ago.

Now, while some of these spots, growths, or blemishes aren't really an issue, others are incredibly painful—they're deep under the skin, and there's absolutely nothing I can do about them.

About two months back, I was in total agony because so many of those areas were swollen, massive, and bright red; I couldn't even sit or walk without just suffering through it, because the pain was constantly throbbing. After about a week, it subsided, and I thought it was finally starting to clear up (though looking back, that might have just been a placebo effect), but now I can feel two subcutaneous "bombs" starting to develop in two different spots, where the pain is slowly but surely ramping up again.

Hygiene isn't the issue here; I don't touch that area at all, I shower every single day—sometimes twice a day—and for the last six months, I've been using a cleansing milk on the area, but nothing seems to have any impact on the situation.

I'm not sure if I've always had these minor breakouts, but I definitely noticed them about a year ago; back then, it was just two or three red spots that would fade away only to pop up again in different places.

If anyone has even the slightest idea of what this could be, I'm all ears, because this is becoming unbearably painful.

One more thing: that part of my body (my glutes and the tops of my legs) is absolutely wrecked. While the rest of my skin, like on my arms, is as smooth and even as a ten-year-old's, this area looks like 100-year-old skin that survived three World Wars and got wounded in every single one of them.

Oh, sure, because that obviously never crossed my mind. But here’s the kicker: my doctor actually wrote it on my referral as acne conglobata—even though what I’m dealing with looks completely different and hurts about five times as much—and now I’m stuck waiting over a month and a half just to see a dermatologist. So, until then, I guess I just sit around and wait 😕

So, this started about ten months ago, yet you’re looking at a month and a half wait just to see a dermatologist? Honestly, given how hard it is to snag an appointment with a skin specialist these days, I can't say I'm surprised that wasn't your first instinct. But here's the thing: if your condition was so severe that you couldn't even sit or walk comfortably, that's considered an urgent medical situation. You shouldn't be sitting around waiting for a routine appointment in a case like that.ž
Plus, let's not forget that there are always private practices and specialists available if you need faster access.

I'm just having a hard time wrapping my head around what you've been doing for the last ten months.

I’m talking about this specific situation right now—the first time this flared up, I didn't even bother seeing a doctor because I just chalked it up to a minor breakout or some stubborn acne. And honestly, the "urgency" factor isn't even my call to make; the receptionist at the clinic just looked right through me and told me they couldn't get me an appointment until mid-February, regardless of whether I'm standing there in agony or feeling fine.
