Password management struggles

I probably should've found an existing thread for this, but I couldn't track one down anywhere.

I’m planning on swapping out my PC today—problem is, I don't have a single password memorized... Facebook, Google, the whole works.
Is there actually a way to pull these passwords before I ditch the old machine, or am I stuck resetting everything from scratch?

If you dig through the settings of any web browser, you'll find a section dedicated to your saved passwords. You can actually go a step further and set everything up to sync those credentials across your devices, so once you log in on a new machine, everything just pulls through automatically...
