On Monday, I dealt with such intense lower abdominal pain that I actually had to take an Advil to manage it (it wasn't menstrual cramps, as my period ended four days ago). I went to see my doctor the following day to provide a urine sample. While the pain has subsided somewhat, it hasn't vanished; instead, it has shifted into this constant feeling of bloating, and I find myself needing to use the restroom much more frequently than usual. What really puzzles me, though, is that my Pap smear from just last month showed mixed flora along with reactive cellular changes linked to inflammation, including some cylindrical epithelial clusters, yet my gynecologist simply told me to come back for a follow-up in six months. Why would they suggest waiting that long given these symptoms?

My urinalysis results came back as follows:
SPECIFIC GRAVITY: 1.025 (1.002 - 1.030)
PH: 6 (5-6)
ERYTHROCYTES: trace
LEUKOCYTES: 5-10
CELLS: many squamous epithelial cells
BACTERIA: moderate
MUCUS: moderate

I won't list the rest since everything else fell within the normal range. I should also mention that I was prescribed Advil on Tuesday, but I am still experiencing that lingering discomfort in my lower abdomen.

If anyone is able to offer some insight or help, I would be incredibly grateful!🙏🙏

Based on this description and the urinalysis results, it looks like you're dealing with a bladder infection. Are you taking Advil at 125 mg twice a day?

No, I’m actually taking 500mg twice a day.
So, you don't think it's related to anything gynecological?
And why am I feeling this slight discomfort near my right ovary now, when I usually only experience that during ovulation, which hasn't happened yet?

That sounds like your bladder. Honestly, it's also possible you've caught a chill in your ovaries. Once you finish the antibiotics, go see your OB-GYN for some swabs. And make sure you take some Fluconazole so you don't end up with a yeast infection from the meds.
For those side pains, you can just grab some Motrin or Ibuprofen; they're basically the same thing.

I’d like to weigh in on this discussion—my most recent bout of cystitis escalated into acute pyelonephritis, which is essentially a kidney infection. I just wrapped up a ten-day course of Advil, taking 500 mg twice a day. My main concern now is how to prevent something this severe from happening again. I deal with these symptoms relatively frequently, maybe a few times a year, but I usually manage to clear them up with some cranberry juice or herbal tea. This time, I honestly thought I had it under control, only to wake up a week later with a 104-degree fever. It turns out E. coli was the culprit. If I submit a urine sample for a culture the very first day I feel symptoms, it takes about eight days to get the isolate and the antibiogram back, so what am I supposed to do in the meantime? Does it actually make sense to jump straight onto antibiotics blindly, knowing how resistant E. coli can be and the risk that the medication might not even work, or worse, that I'm just helping other bacteria develop resistance?

I went through this whole bladder infection nightmare about ten years ago. It was happening basically every month. I’d walk around my apartment barefoot, and then the next time I had to go, it was pure fire—visible blood in my urine and everything. Of course, I went through the whole drill: antibiotics, cranberry juice, parsley tea, you name it. Back then, cranberry wasn't even the big thing it is now. Honestly, I finally kicked it by just focusing on my immune system. I cleaned up my diet a bit, took some basic vitamins (nothing fancy or overpriced), and most importantly, I started walking barefoot more often. I started at the beach, moved to the hardwood floors during the summer, and eventually transitioned to tile. Apparently, that was the "secret sauce" for my inflammation, because I haven't dealt with an infection in years. 🤷
