I’ve been dealing with this nagging pain in my perineum for quite a while now. Sometimes it hits right after I ejaculate, sometimes after a bowel movement, and other times it just shows up out of nowhere at random. I’ve already gone through the whole gauntlet of testing—urethral swabs for chlamydia, ureaplasma, and mycoplasma, plus urine and semen cultures—but everything came back totally clean. Urinating feels completely normal, too.
My doctor mentioned it might be an issue with my prostate. Honestly, I’m dreading seeing a urologist because I know they’ll just send me on a marathon of more tests, and by the time I actually get answers, months will have passed.
Does anyone have any advice on what I should do? I'm open to suggestions for medications or even some natural remedies if they actually work. Also, if I absolutely have to see a specialist, could anyone recommend a solid urologist?

Well, nettle tea is always a decent option to try... by the way, you should look into some pelvic floor exercises for the prostate...

Is that pain in your "groin" area located right below the testicles? Does it hit more on the left side, and does the ache radiate into your groin? Does physical exertion make it worse? Also, do you actually feel the pain inside the testicle itself?

To be honest, an urologist shouldn't need to run a mountain of tests. If you bring in those bacterial culture results, get a quick ultrasound, and go through a digital rectal exam, they should have a clear picture pretty quickly... It doesn't necessarily have to be the prostate.

P.S. Frequent ejaculation is actually quite beneficial for prostate health.

I don't know much about those symptoms, but since testicular cancer can move incredibly fast, I’d head straight to a urologist if I were you. Not trying to freak you out, but given how long this has been going on... check out this site you might find something useful there.

So much for that stereotype about women being overly dramatic. Honestly, swiftscout8, you're better off overreacting and seeing a urologist than playing it cool and ignoring a problem that might actually be there, God forbid. I lost a close friend to testicular cancer because he thought he could just brush his symptoms aside.

It’s nothing to do with my testicles—they never ache, there's no connection to physical exertion, and the pain doesn't radiate anywhere else. The discomfort is localized right in the perineum, specifically the area between the penis and the anus. Neither the penis nor the anus itself hurts at all.
I went through the recommended page, but honestly, none of those symptoms seem to fit what I'm dealing with.

From what I’ve dealt with, that sounds like prostate issues. I went through those exact same symptoms, even though my tests always came back clean—no bacteria in my urine or anything. I can't even remember the specific name of the pills the doctor prescribed, but after about 5 or 6 days on them, everything cleared up.

That pain is actually located behind the testicles. I don't think I explained it very clearly in my last post.
Does anyone have a recommendation for a good urologist? Ideally, I’d prefer a woman; I'm just a little more comfortable being examined by a female doctor than a guy.
Look, I'm not gay or anything, it just feels less awkward with a woman:-)🙂

I honestly don't think you need to sweat this too much. I get those same random pains here and there myself, and I can never really pin down what's causing them, but they eventually just fade away. To be fair, for me, it usually flares up right after I ejaculate—kind of feels like those prostate contractions we used to talk about back in my college biology days...

I’m still dealing with the same old issues. I haven't actually made it to the urologist yet, but I did head back for some more blood work and urine tests, and honestly, everything came back looking perfect. While I was killing time, I was flipping through this book on prostatitis: acute and chronic, and I stumbled upon something pretty interesting. It mentioned that the root cause of prostate inflammation might actually be linked to sinus issues—which is wild, because I've been struggling with chronic sinus problems for about three or four years now.

Since you've been dealing with this stuff for quite a while now—and honestly, it sounds like classic prostate issues—I’d really suggest just biting the bullet and seeing a urologist. Getting an ultrasound and a quick digital exam should be enough to start with, just so you actually know what we're dealing with. Once you have a real diagnosis in hand, figuring out a treatment plan becomes a whole lot less of a guessing game.

As far as natural remedies go, they can actually be pretty effective for prostate stuff if you use them right. Take nettle, for example. You can't just grab any old thing off the shelf; you need to make sure whatever supplement or tea blend you're using is made from the root, not just the leaves. Using the leaves won't do much for you.
I'd also suggest trying some suppositories for about ten days or so—specifically ones formulated with essential oils like chamomile, myrtle, and LEDUM. It might help take the edge off.

Thanks for the advice! Do you happen to know where I can find those types of suppositories, and can I pick them up without a prescription?

So, I finally made it to the urologist today. He went over my symptoms and walked me through my results, then recommended that I get a renal ultrasound and a prostate expression culture. That’s basically the whole visit—he didn't even perform a physical exam.
The culture part needs to be done at the Mayo Clinic.
I was wondering if anyone here has gone through this specific test before? Specifically, how long does the doctor usually have to massage the prostate before the secretion actually starts flowing through the urethra?
I've heard from enough people that this procedure is definitely not exactly a walk in the park.

I had those exact same symptoms when it turned out to be a varicocele... It’s a relatively minor procedure, and you’ll feel like a brand-new man within a week 🙂
Do a little digging online, but definitely go see a urologist...
