I'm 20 years old, and my period usually lasts about a week without much delay. But every single time I get it, I fall into this horrible depression, and my PMS symptoms start up to 10 days before my cycle even begins. I don't know, I'm not sure if it's actual clinical depression or something else. I just don't know what to do.
I've definitely had episodes where I just can't bring myself to get out of bed. This time feels like the worst one yet. Usually, I deal with stomach cramps at the start of my period, but not this time. It's just been this crushing exhaustion. My period started on Thursday, and since then, I haven't been able to do anything. I've just been shut in my room all day, unable to focus on studying, even though I have a midterm tomorrow.
About two months ago, I actually self-harmed for no real reason. Maybe this depressive state is tied to my PMS and my period, but I can't keep going through this every month. Should I go see a doctor or a gynecologist?

1. First off, you’ve gotta head to your OB-GYN just to make sure there isn't some underlying physical issue going on
then,
you should probably hit up your primary care doctor on the same day to get a referral for a psychiatrist, and you'll need to be pretty upfront with them about what's happening:

"...About two months ago, I started cutting myself, and honestly, there wasn't even any specific reason behind it...."

Hey everyone! Bringing this thread back to life 😁

Basically, I’m dealing with the exact same thing as the OP. My PMS symptoms have been incredibly intense over the last year.
I’m not really the type to obsessively track my cycle on a calendar since there isn't much reason for me to worry about it (I'm not sexually active and my periods are regular), but I can almost always feel a "click" in my brain that signals my mood is about to shift. The last time it happened, it was brutal—honestly, I think I slipped into a pre-depressive state. I don't want to sound like a hypochondriac or someone playing doctor, but everything I read online matched my symptoms perfectly. I even felt like I had a few panic attacks during that stretch.
The weird part is that I know the second it stops. Literally an hour after the bleeding starts, 😁 and within half a day, I feel like myself again.

Has anyone else gone through this? If you have, how did you handle it—or did you even find a way to fix it?

Some women are incredibly sensitive to fluctuations in estrogen and progesterone, whether those hormones are naturally occurring or introduced through birth control or hormone replacement therapy. For these individuals, the physical and emotional shifts can be quite intense. In some rare cases, PMS can manifest so severely that it mimics the symptoms of an anxiety or depressive disorder. You might want to look into Premenstrual dysphoric disorder. Unfortunately, many gynecologists aren't fully aware of just how profoundly estrogen and progesterone can impact a woman's mental health. It would be ideal if gynecologists and endocrinologists collaborated more closely to manage these hormonal treatments and mitigate these effects.
