I need some help making sense of this situation regarding my girlfriend's father.

He’s 45, works a high-stress job, and smokes and drinks. Last year, he was at risk for a heart attack, so they placed a stent in his pulmonary artery via a femoral approach. Now, nine months later, he’s dealing with intense pain in his hip and leg. We took him to see a doctor, but he’s incredibly stubborn about following advice. His current medications include Lipitor, aspirin, Plavix, and Coreg.
The report states:
Hip MRI, T1 coronal and transverse, T1 stir coronal, T2 tse transverse, and T2 tse sagittal. An irregular zone of altered signal intensity is visible in the head and neck of the left femur. The signal is moderately low in T1 weighting and high in T2 and STIR weightings, suggesting signs of bone edema. The superior and lateral convexity of the left femoral head is demarcated by a line showing low signal intensity across all sequences. There are signs of joint effusion in the left hip. The articular surfaces of both hips are congruent, though the intra-articular spaces are slightly diminished. Other visualized bone and soft tissue structures show normal morphology.

Conclusion: Aseptic necrosis of the superior and lateral portion of the left femoral head.
Signs of bone edema in the left femoral head and neck.
Left hip joint effusion.

Everyone is recommending surgery and rest. However, his job requires him to be on his feet every single day; he simply cannot take time off.
Do you think recovery is possible without surgery? The doctors don't seem optimistic.😢( 😢😢😢😢😢😢
