Basically, you handle the M13p just like you would for activating a registration—you really only need to fill in the business start date. Then, obviously, you fill out the M1P (the one covering the owner's insurance basis...) Oh, and I usually attach the official seasonal business approval documents, though honestly, I don't think it's strictly mandatory...
Meanwhile, Medicare & Medicaid Services told me today it's M1P and M11P.
