If they’ve isolated Escherichia coli, there really should have been an antibiogram performed on that specific strain. In that scenario, you’d want to stick with whichever antibiotic(s) the isolate actually shows sensitivity to; otherwise, Vancomycin is essentially the last resort used in those cases.
