Does anyone happen to know what this actually is, and if surgery is usually required... it's located in the brain...

I also looked it up and it seems benign... but I'm wondering how risky this actually is and if surgery is on the table. The patient is over 30... and the cavum vergae measurement is 18mm...

I feel like you're gonna have a hard time getting any real answers this way. Maybe try writing out a bit more detail? Like, who are we talking about, their ages, what kind of medical findings they've got...

From what I understand based on my own experience, a cavum vergae is essentially a malformation that rarely presents any actual clinical symptoms.
It can lead to hydrocephalus or aqueduct obstructions, which would likely necessitate surgical intervention in those specific cases.
You mentioned it measures 18mm, but you didn't specify if it's causing any issues. How was this discovered? Was it due to certain symptoms, or just a random finding during a CT scan for something else?

It was because of symptoms... discovered after having another neck surgery...
The symptoms are:

feeling sleepy and fatigued
Headaches
Irritability, which seems to be getting worse...
Loss of appetite
Lethargy
Nausea,
slowed thinking...
trouble concentrating,
it feels like my brain is just fading away... all I want to do is sleep... even though my blood pressure and blood work are totally fine...
