Honestly, the rule stating "insulting other members is prohibited" isn't ambiguous or confusing at all. There isn't some hidden loophole anywhere saying it's okay to insult someone just because you don't explicitly mention their username.

Look, like I already mentioned, it really comes down to the tone of the nickname distortion. If it's being used to be offensive or provocative, then it's against the rules. If you see someone twisting a name to be disrespectful, just report it and the moderators will step in to decide if a sanction is necessary.
