Regurgitating food: Is this normal?

So, for about a month and a half now, I’ve been dealing with this incredibly annoying issue where liquid and food basically shoot right back up into my nose. It actually started out just with liquids... you know, like that weird thing that happens when you laugh while drinking and everything goes down the wrong pipe? Well, it's like that, but way more constant. And honestly, it isn't even like classic acid reflux where stuff comes up from my stomach or esophagus; it literally feels like it happens the exact second I swallow, right as the food is passing through my throat. It doesn't happen with every single bite, but it happens at least once during every meal. I mean, it even happens when I'm just swallowing saliva, though that's not quite as bad as when it's actual food or coffee or juice, because that constant splashing of liquid into my nasal passage makes the area where my nose meets my throat sting like crazy. And look, I am trying! I eat so slowly, I chew everything into tiny little bits, and I take these tiny, careful sips of water, but it still happens.
I went to see my primary care doctor about it, and she just gave me a referral to an ENT specialist, saying there could be maybe five or six different things causing this. I tried asking her specifically what she thought it might be—I asked her several times, really—but she just kept looping back to the same thing, telling me to just go see the ear, nose, and throat doctor first and then we'll figure it out. Does anyone have any idea what this could possibly be? I'm 36. Thanks so much! 🙂

In medical terms, what you’re describing is nasal regurgitation—essentially, swallowed contents traveling back up through the nose.

There are plenty of different reasons why this happens. Without a proper exam, pinpointing the exact cause is impossible.

I think your doctor made the right call by referring you to an ENT specialist. They’ll likely check to see if there’s a structural issue, such as a malformed or short soft palate. If the palate can't seal off the nasal passage properly during swallowing, food ends up where it doesn't belong. They'll also examine the oral and nasal parts of your throat. Is it possible you had some kind of surgery in that area recently? That could be a factor.

If the ENT doesn't find anything, the next logical step would be seeing a neurologist. Certain neurological issues can weaken the soft palate, preventing it from closing off the nasal cavity. There are many possibilities there, but I wouldn't get ahead of myself just yet. Let's wait for the ENT's findings first.

So, I had my tonsils ripped out back in the late '90s, and honestly, I haven't thought twice about it until about a month and a half ago when things started acting up. I’m actually heading back to see my ENT specialist in a week, so hopefully, I'll finally get some real answers then.

Beyond just seeing an ENT, you should probably check in with a gastroenterologist too. I’m thinking you might be dealing with Republican Party issues.

It’s not GERB because we aren't talking about food that's already been swallowed, and I don't have any gastrointestinal issues to speak of. This happens literally the split second you swallow, right when the food is supposed to be sliding down your throat into your esophagus, but instead, a chunk of it just shoots straight up into my nose.

So, I headed over to the ENT specialist today, and honestly, my head is spinning because besides this weird little indentation in the mucosal lining right near the sphenoid opening—which, let’s be real, I have absolutely no clue what that actually means since I just copied it straight from the doctor's notes—everything else seems fine. I have to go get a swallow study X-ray done now, but the doctor was dropping these hints that it might actually be something neurological instead? Like, what does that even imply? Does anyone know what they could be getting at? And also, is that swallow test going to be super uncomfortable or what?
