Back acne: What works?

My son is dealing with these massive, inflamed breakouts covering his back, shoulders, and upper arms.
It’s been an ongoing struggle for years now, and nothing seems to settle them down. They first flared up right as his teenage acne was clearing off his face, almost as if the inflammation just migrated downward. He’s thirty years old now, and the issue hasn't budged.
Interestingly, his skin clears up significantly during the summer when we’re down in Florida; the constant sun exposure and swimming in the salt water seem to provide some relief.
He’s quite diligent about hygiene, showering at least twice a day, yet none of our efforts have paid off. We’ve cycled through various acne cleansers and topical creams, but they haven't made a dent.
At this point, it feels like something systemic—something originating deep within his body—which has me wondering what the underlying cause might be.
I should also note that these lesions often look less like typical acne and more like inflamed cysts.

Yeah, same here. Only difference is I'm 18... and ever since puberty hit, I've been stuck with them. I have so many, and they all have this weird purple tint... they stretch from the top of my latissimus dorsi all the way down to my shoulder blades. It's the exact same thing where everything clears up once I get to the beach... the salt air, the sun... basically tanning... :/ I just don't get it.

Something like this: A friend’s wife actually dealt with this—she used some kind of solution she picked up at a local drugstore. She was super consistent about squeezing them out and disinfecting the area with it... and after two years, they were totally gone.

MRSA??? I highly doubt it...

My husband has these exact types of breakouts scattered across his scalp right now. We’re currently on our tenth attempt at different shampoos and creams... just hoping something finally works. 😁

That first link shows an atheroma, not some random pimple. Trying to squeeze an atheroma at home is easily one of the dumbest things you could possibly do, not to mention how dangerous it is. 🙂

Anyway, there are dermatologists for skin issues. Is nobody here actually seeing a specialist? Not a single person has mentioned getting a professional checkup.

And honestly, I have no idea why I let myself watch that first video. It made my stomach turn. 🤮
