Has anyone here actually gone through the process with them—I mean, specifically at the Vatican City facility—and how does that whole setup work? Can they actually deny sick leave? My doctor mentioned that if I’m out on medical leave for more than six months, the law requires me to undergo an evaluation there. So now I have to go, and I'm honestly wondering what this whole thing is about and if they're just looking for excuses to be difficult.

Legally speaking, the primary physician is required to submit an opinion to the Ethics Committee once a leave exceeds six months.
The Ethics Committee then decides whether to extend the leave, determine if the treatment phase is complete, or assess if the patient meets the criteria for disability retirement. There are many variables involved, which suggests your doctor is simply following standard procedure.

I’m struggling to wrap my head around the logic here—if they supposedly have the authority to terminate medical leave as you claim, then why wouldn't they also have the right to extend it? It just doesn't seem to follow.

They simply provide an opinion, and their opinion is subject to change...

The insurance provider you deal with regularly tends to mirror their stance...

Essentially, if they determine the illness is still in the treatment phase, the insurer will extend coverage based on your medical records, following standard protocols and specific guidelines...

If they decide to extend my medical leave, then why on earth am I still being forced to show up at the clinic?

The doctor provides an assessment regarding whether disability has been established, if recovery has occurred, or if further therapy is necessary...

Essentially, a full evaluation happens every six months or annually. The Social Security office issues regular findings on whether the leave should be extended or not...

In short: one is a comprehensive assessment of your condition, while the other is a determination of your immediate need for leave...

That is simply how the system works; there is no use overthinking it.
