How to request a partial ban?

As the title suggests, I’m looking for some clarification on how partial bans are actually issued here. Given that there are seven-day and monthly bans, it seems there's a clear escalation in penalties—and presumably an accumulation of points—that allows one to bypass certain stages. How does that process work?

Are the criteria within the individual PDFs standardized when it comes to issuing a partial ban?

Who handles appeals for partial bans?

On a side note, what is the actual path to becoming a moderator on this forum?
