I’m not too sure about the situation in NYC or Miami—honestly, I'm a bit out of the loop there—but maybe just give the Medicare legal department a call? Or, if you're feeling adventurous, head down to their office over on Jukic Street, or wherever happens to be closest to you. Other legal routes include paying for private insurance yourself, or having someone hire you for a single month just so you can "resign" and then re-apply for free coverage within the thirty-day window... though that would still cost the person hiring you a chunk in taxes. What a headache!

That was true a year ago when I was dealing with my Medicare paperwork—and honestly, those outdated timelines on the official websites haven't changed one bit. Someone might turn a blind eye and let an application slide through, but they certainly weren't willing to do that for me.

You’ll have to pay up if you don't head over to Medicare to sort out your health coverage within those specific windows—like three months after graduation or one month after leaving school—as outlined on their official site.
The truth is, this has nothing to do with the stock market; per the Constitution, you're entitled to free healthcare—provided, of course, that you actually follow the deadlines.

I usually do all my talking while I'm getting undressed, while I'm waiting for them to come over to the table, or even while they're performing the exam—thankfully, most doctors are polite enough to step behind their screen to their desk immediately after finishing, which gives me plenty of time to keep chatting (if I actually have anything to say). But, oh well, like I said—everyone has their own way of doing things.

Honestly, I find the idea of wearing a skirt just because you're at the doctor a bit silly—I mean, you're already showing them your most private parts, so why bother worrying about half your backside?
I always just show up in pants. My nurse stays in the other room anyway—and even if she does come in, the exam table is tucked behind a curtain along with the changing area, so everything is pretty private.
Of course, if you're more on the shy side, do whatever makes you comfortable...
As for the specifics of a first-time exam, I'll leave that to someone else—hopefully someone who knows more than I do!
