Hi everyone, I’m currently in my 11th week of pregnancy (10+5) and I went in this morning to get some blood work and a urine test done.

The results just came back and everything seems to be within the normal range, except for one thing mentioned in my urinalysis regarding salts: an unquantifiable amount of amorphous phosphates.

Does anyone know what that actually means? 🤷

Thanks.

Can someone help me make sense of these results?
Urinalysis—
Appearance—slightly cloudy (clear)
Color—yellow (pale yellow)
Protein—trace (negative)
Leukocytes 12 (normal is up to 2)
Erythrocytes 2 (normal is up to 2)
Squamous epithelial cells 2 (normal is up to 1)
Small epithelial cells 0
Hyaline casts 0
Non-hyaline casts 0
Crystals 0
Salts 0
Bacteria—quite a bit
Yeast 0
Mucus—some presence
So, if I'm reading this right, we're looking at some kind of inflammation here?
But how am I supposed to figure out where exactly the infection is?

Is this some kind of infection based on what I'm seeing?
And how can I tell where the inflammation is coming from?

It looks like a bladder infection. Your report shows a high amount of bacteria, and urine should be sterile. You really need to get a urine culture with an antibiogram done so you can identify the specific strain and figure out which antibiotic will actually kill it off most effectively.

It basically means you probably aren't drinking enough fluids. Phosphate salts tend to precipitate when urine is alkaline, so you really should be aiming for about 2.5 liters of fluid a day. You might also want to try eating more meat (just skip the organ meats for now) and cutting back on plant-based foods and dairy until things settle down. If this keeps happening, it could potentially point toward a calcium or phosphorus metabolism issue somewhere in your system.
But honestly, most of the time, it’s just because women aren't staying hydrated enough.

Your IgG came back positive, which means you've got immunity and you're protected. You were exposed to the virus at some point, though it's hard to say exactly when.

The IgM is negative, even if it's hovering near that "upper limit of normal." Given what you told me, it's a bit confusing, but I’d still interpret it as negative.

Just keep in mind that under certain circumstances, the virus—which stays in your system forever—can flare up again, and that's when you deal with shingles.

Best regards. 🙂

Thanks a ton for the reply!

My GP gave me some antibiotics today. I'll finish this course first, then I'm heading in for the urine culture and swabs...

Ugh, I can't stand it when they just play guessing games like that.🙄
Next time this happens, you really need to dig your heels in and insist on the urine culture first. Handing out antibiotics based on a hunch is basically playing Russian roulette with your health. If they don't hit the exact right strain of bacteria, you're just nuking your immune system for no reason. Then you end up needing *another* round of antibiotics to fix the damage, which wrecks your immunity even further, and before you know it, you're dealing with fungal infections and stuck in a vicious, endless cycle.

Looks cloudy.
Yellow tint (pale yellow).
pH levels are acidic (4.6–7.5).
Specific gravity: 1.020 (Reference range: 1.005–1.030).
Protein levels are at 0 (normal range: 0-3).
Glucose levels are normal. Everything looks good here.
Ketones: 0 (Range: 0-3).
Bilirubin at 0 (range 0-3). Perfectly normal.
Urobilinogen is within the normal range. Everything looks good here.
ERCs: 0 (Range: 0-3)
HB levels at 0 (range 0-3).
White blood cell count is at 1 (normal range 0-3). Looks normal to me.
Nitrites are negative (0-2).
URINE SEDIMENT
White blood cell count is sitting at 15-20. That’s high.
The bacterial load is massive.

The day before this test, I had a urine culture done, and it came back sterile. I was dealing with a vaginal infection and used some Arilin rapid wipes. Based on these results, do I actually need an antibiotic for my urine?

Next time this happens to you, don't let them off easy—insist on a urine culture first. Throwing antibiotics around blindly is basically playing Russian roulette. If you don't hit the exact bacteria they're actually dying from, you're just nuking your immune system for no reason. Then you end up needing *another* dose of antibiotics because your defenses are shot, and before you know it, you've picked up a fungal infection and you're stuck in this endless, miserable loop.

That's what I was thinking too...
So I guess I just have to hope their "psychic powers" lead them to the right med... :-/

I’d love to get some input on this:

Back before I even got pregnant, I was seeing protein in my urine (+). Once I hit my third month, my OB/GYN sent me to see a urologist for advice. Both the urologist and the nephrologist told me to stick to a low-sodium diet, cut back on protein intake, check my blood pressure daily, and keep a close eye on any swelling in my eyes, hands, or legs.
Now I’m heading into my eighth month. Normally, I have pretty low blood pressure—usually around 110/60—so even now, it hasn't climbed above 130/70, even when I'm exerting myself. My hands don't swell at all, and my legs only swell occasionally if I’ve been stuck sitting at my desk for a few hours without walking around. Mostly, though, everything seems fine.

Just as a routine check, I mentioned the protein issue to my doctor, and they ordered a 24-hour urine collection. Here are the results:

(I assume these are the blood work findings)
Urea 0.9 (Ref: 1.7-8.3 mmol/L)
Creatinine 74
Total Protein 67
Albumin 36
Globulin 31

Total protein in 24h urine 0.052 (Ref: 0.080-0.149)
dU Protein conc. 0.021 (Ref: 0.050-0.119)

Creatinine clearance 0.78 (Ref: 1.0-2.8)
dU Creatinine conc 2010 (Ref: 7000-25000)

Diuresis 2490 (Ref: 1000-2000)

Are these numbers normal for someone following the specific diet I was prescribed? I have to wait a few days for my doctor to actually review them, but I have to admit, seeing these low values is making me a bit uneasy.

Thanks in advance.

Edit: Just one more thing. Throughout the day, I drink at least 2 liters of water, about half a liter of tea (chamomile or cranberry—I've been dealing with a cold for the last couple of months), plus a glass or two of lemonade or fresh juice. So, I'm hitting at least 3 liters of fluid a day. Could that be messing with the results?

The nurse at the hospital where I was monitored (given the risk of recurring preeclampsia and HELLP syndrome) told me that by flushing your system with water, you can actually get better readings on the 24-hour urine and creatinine clearance... based on your diuresis, if her claim is right, it's possible that's what pulled your otherwise normal levels down. Personally, I think around 2.5 L of fluid a day is plenty. You shouldn't overdo it.
If your platelets and liver enzymes look good, I don't think there's cause for concern. If you suddenly start gaining weight rapidly or notice facial swelling, that could indicate an issue.

EDIT: God forbid anything actually goes wrong and an emergency delivery becomes necessary; at 8 months, there shouldn't be major issues with the baby. My second child was born at 34 weeks and everything turned out fine—the birth weight was solid enough that there was no need for an incubator or a warmer.

Antibiotics are prescribed based on the antibiogram from a urine culture report. Honestly, I can't wrap my head around how you can have a massive amount of bacteria and elevated leukocytes in one test, yet have a completely sterile culture result. You really ought to redo the culture, just to be safe.

Thanks for the reply. I don't get it either. It makes zero sense—I’m still hitting the bathroom every single minute, the stinging is constant, and now I’ve started getting this sharp pain in my kidneys. I have no idea if my doctor will even let me redo the urine culture, since according to those results, everything is supposedly fine. :/

Look, you can always just head to a private lab. But listen—you have to be extremely careful about how you collect the sample! You need to wash up first, wipe with sterile gauze, and then catch the midstream portion of your very first urine of the morning. Personally, doing it in the bathtub is the easiest way to avoid a mess... at least for me. 😬
Taking some Neo might help take the edge off those symptoms.

EDIT: Since it's the weekend, don't rule out heading straight to the ER. They can run the tests immediately and get you started on treatment right away 😉 I've had to pull that exact move several times myself.

If you brought me lab results like those, I’d personally just hand you an antibiotic right then and there... Why? Because you've got leukocytes and bacteria sitting in your urine sediment—which isn't a smoking gun on its own—but you also have a positive leukocyte esterase result in your urinalysis, which is basically that part showing the white blood cells... And given that you actually have symptoms, honestly, I wouldn't even bother ordering a urine culture unless empirical therapy fails or some other complication pops up, because frankly, it's just not worth the hassle. A single pack of Ciprofloxacin or something similar, which usually clears up uncomplicated cystitis in about four or five days, is way cheaper for the healthcare system than running cultures and subculturing samples, especially since that process takes several days here anyway... So, in my book, you should just get the antibiotic started while the inflammation is symptomatic and wait for the results to catch up... 🤷

Can someone make sense of this for me...

Lab hematology... just more numbers to stare at...

SE 42
erythrocytes 4.53...
RDW 13.4... fine.
Hemoglobin 131... alright.
Hematocrit 0.398...
MCV 87.9... alright.
MCH 28.8... fine I guess.
MCHC 329... nothing special. Just numbers on a page.
Platelets at 352...
WBC 6.7...
Neutrophil granulocytes at 60.6... nothing crazy, I guess. Just another number on the page...
Lymphocytes at 30.4...
MID 9.0

Metabolites and substrates...

glucose 5.2...
Bilirubin 12.4... damn.
Urea 4.1...
Creatinine 71...
Urea 278...

Enzymes...
AST 13... fine.
ALT 12

Urinalysis physical and chemical profile...

Appearance—clear...
Color—light yellow...
pH 5...
Specific gravity—1.020...
Glucose levels look normal...
Bilirubin at 0... nothing there...
Ketones at 2.5 mmol/l... not great...
RBC/Hb... zero...
Protein levels... zero.
Urobilinogen levels... normal.
Nitrites—0...
leukocyte 0...

Urinalysis... microscopic sediment exam.

erythrocytes 0-2...
leukocyte 2-4...
squamous epithelium 2...
Mucus 3
Soli 1

My sedimentation rate is up... sitting at 42, while the lab says anything over 24 is out of range. Just great...
Still waiting on those urine culture results... Is it actually possible for them to come back messy even though my GP told me everything looked totally fine?...
Leukocytes are at 2 - 4, ref range is 0 - 2... barely even worth mentioning. 😕

Theoretically, sure, it’s possible—but I honestly don't buy it. It’s way more likely she just had some kind of nasty cold over the last couple of weeks and that’s what bumped up the ESR... Besides, trying to read an ESR reading on its own is a total nightmare because so many different things can spike it, and it’s pretty much useless as a specific indicator anyway, so if I were you, I wouldn't be losing any sleep over it... By the way, looking at your results, I see you've got ketones in your urine despite having normal glucose levels in both your blood and urine (which usually points toward ketogenesis from fasting), so there's a chance that elevated sedimentation is actually being driven by that too... 🤷

Just adding some more context to my diagnosis. It’s weird that my urinalysis looks fine because I've been having trouble peeing, plus last month my kidney was killing me... had a fever and chills too.
For the last two weeks, my ankles have been super swollen, like they're just full of fluid. They're red and sore if you touch them. 😢
It was actually making it hard to walk lately, especially first thing in the morning when I get up. I also had edema around my knees and shins, but that cleared up.
Now my legs and shins just ache, mostly when I'm sitting for more than 40 minutes.
Doctors are clueless. I've already hit up two different hospitals and my primary care doc. 🙄
Throat swab came back clean, lungs are fine too.
I was really hoping it was something urinary-related, so fingers crossed the urine culture actually catches something. Is it possible for a standard urinalysis to look normal but the culture shows an infection?
Read somewhere that ESR can be high with rheumatic fever or rheumatoid arthritis—basically just any kind of inflammation in the body. 😕
I'm in my early 20s.

Also, what's the deal with RDW? Mine is 13.4, but the ref range is 14.6 - 16.5.
