How do I request my old medical records?

I need to track down some old medical records from back in '94. Does anyone know if they
still keep stuff like that in the archives, and what's the actual process for requesting them?

And even if they aren't being kept—though I'm pretty sure they are—everything should be logged in the medical record anyway.

They seriously need to provide a full medical history...
