Has anyone here ever dealt with a cracked health insurance card and needed a replacement? They’re telling me I have to fill out Form 1, attach a photocopy of my driver's license, and head down to Medicare & Medicaid Services with the broken card to hand everything in—marked clearly as a "replacement" on the form. Is that really the process?

Hey there!

I really need some help here!

I need to update an employee's last name on their health insurance because they got married, and I need to get them a new card. But honestly, how the hell do I actually do this? Am I supposed to go through Medicare & Medicaid Services? I’ve been messing around with their website at lana.mirovinsko.com, but I’ve never dealt with a situation like this before. Do I have to deal with a mountain of paperwork and wait in line at a local office?

Either way, you'll need to update it with Medicare & Medicaid Services too—just fill out the necessary form on the lana.mirovinsko.com portal, upload the marriage certificate, and they'll process the change. Once that's done, just print it out and have her take it down to the insurance office to see what specific form they require for the swap...

Someone at the Medicare & Medicaid Services office mentioned to me recently that they require us to hand-deliver every single update in person. Apparently, even when you just need to correct some basic information, you still have to file that T1 form manually...

I only ever update my info through lana.mirovinsko.com, and so far, nobody from Medicare & Medicaid Services has ever come looking for updates. If they’ve switched to an automated system, I honestly assumed everything would sync up automatically once I updated it on the Social Security site...🤔

The word on the street is that only enrollment and disenrollment are automated. Everything else still requires paperwork filed directly with Medicare & Medicaid Services.

The folks at the US Census Bureau told me that sign-ups and cancellations are all handled electronically via lana.mirovinsko.com, which then supposedly syncs right over to Medicare & Medicaid Services. But for anything else? Apparently, you still have to hand-deliver paperwork because their electronic systems aren't quite there yet. If you need to change your last name, for example, you have to bring in a stamped, signed employer form along with a copy of your ID. They said the new insurance card gets mailed straight to the employer's office.

Here’s my firsthand experience with this. We actually went through a name change due to marriage just two days ago. We handled the paperwork with the Social Security Administration first, following the standard procedure. Then, we logged into the Medicare & Medicaid Services portal, where you can update your last name directly. Just a heads-up: if you move, the address updates automatically once the DMV processes it, but you still have to manually hit save to make it stick. So, you simply enter the new surname, click "save changes," and your employee data will reflect correctly on the PDF forms. You don't even need to visit a physical office for anything.

Let me correct myself here.

The Medicare Portal handles enrollment, family dependents, and updating an employee's basic info, like their last name or home address.

However, when I had to update my business headquarters, the company name, or my shift hours, I had to show up in person.

Unfortunately, I don't use the Medicare app. I'm guessing setting up an account would take forever...
EDIT: What I meant was I've never used any of those Medicare online services. I've got my Chase card, and I use that for Lana Milovinski and the IRS.

So yeah, looks like I'll have to handle this the old-fashioned way by mailing in the T1 form.

A company with zero employees—do I still have to file anything with Social Security or Medicare & Medicaid Services? Does anyone have any actual experience with this...

Nope, you don't have to. You can register the business with Medicare & Medicaid Services just to get a registration number on file, but if you aren't hiring anyone, it’s totally optional... Once you finally bring on your first employee, that's when you handle the filings for both the business and the worker at the same time...

Tomorrow, I intend to deregister an individual from both their pension and health insurance records. My plan is to process this through the online pension portal using the standard electronic filing form. Does this procedure necessitate a separate submission via the Medicare portal, or will I receive an automated confirmation email from the health insurance provider once the deregistration is finalized, much like my previous experiences? 🤔

They don't send confirmations via email anymore. You have to log in to the Medicare portal and download the confirmation there yourself.

So, we are back to dealing with Java issues and all the other nonsense that comes with downloading these documents. Am I correct in assuming that Chrome won't even work because it lacks NPAPI support, meaning I'm forced to use Internet Explorer just to get this done?

This is how I am currently attempting to retrieve the confirmation via the Medicare portal:

Log into the application — select business users — go to view registrations and cancellations — enter the Social Security number of the individual being deregistered — and then the system should automatically generate the confirmation as a PDF.

Is this indeed the proper procedure, or have I fundamentally misunderstood the process?
