They play the "super caring, ultra-engaged" role, hoping the patient will somehow feel obligated to reward them for the drama. 🤮. Not too long ago, I had a patient who took a nasty fall while skiing in Canada and broke his clavicle. It was a classic fracture right on the border of the medial and lateral thirds, but the displacement was huge—about 4 to 5 cm. He was treated in Venice, where they basically just slapped some hardware on him instead of using proper plates and screws. Clinically, the situation was a disaster: he had about a 7 cm defect in the shoulder plateau, significant shoulder drop, and lost about 90% of his range of motion on that side. He ended up coming to me by chance—he’s from California and is an acquaintance of a nurse who works at our clinic—about a week after the injury happened. Since my specialty is small joints and hands rather than major trauma, I tried to reduce it myself. It didn't work. I put him in a rigid Cellacare clavicle brace and tightened it as much as possible to try and gain some length, but I was honest with him: I told him surgery was likely necessary because, without it, there was a massive risk of non-union. I advised him to get X-rays in San Francisco and see a trauma specialist immediately. Anyway, he got the imaging done. Two weeks after the break, there was zero sign of callus formation; instead, the bone ends were just resorbing faster. The orthopedic surgeon who saw him (this guy had recently been in a bad car accident and spent fifteen days in a coma—so, granted, you can somewhat excuse him, though the alcohol didn't help either) looked at it and yelled: "WHAT ARE YOU DOING HERE? WHO ON EARTH TOLD YOU THIS WAS FINE??!!?? EVERYTHING IS PERFECT, JUST GO HOME AND COME BACK IN TWO WEEKS..." The guy came back to show me the results, and clinically, nothing had changed. I eventually recommended he head over to Carmel-by-the-Sea, which is where he finally ended up getting the surgery. After that, I lost track of what happened. 🙂

You can snag an incentive of $22 to help get your walnut orchard started, plus there’s a production subsidy that lets you pull in $0.83 per hectare every single year once things are established. When it comes to the waiting game, standard walnut trees usually take about 10 years to start producing, though if you go with grafted varieties, you could see a harvest in as little as 8 years. A tree really hits its stride and reaches full maturity after about 20 to 25 years. As for the market value, shelled walnuts can fetch up to $20 per kilogram, while the price for walnuts still in the shell goes up to $3.25. If you have a single mature tree in full production, you could potentially harvest up to 30 kg of shelled nuts. On a one-hectare plot with 200 trees, that works out to 200 trees * 30 kg * $20/kg, which equals a yearly revenue of $120000.
Hey everyone, Lately, I’ve been dealing with this really strange sensation—this constant sense of dread and internal restlessness, almost like I'm waiting for something bad to happen or fearing that I might actually die. When I'm fully immersed in work or staying busy, I feel fine, but the second my mind starts to wander, these thoughts take over and I just can't shake them. I’ve gone through some medical tests and everything came back normal so far, and I have an MRI scheduled for tomorrow; I'm really hoping for good news. Currently, I'm taking Sulpiride and Pergen Forte. If anyone else has dealt with these kinds of symptoms, please reach out...😢 😢 😢

Does anyone know where I can go to train Brazilian Jiu-Jitsu in San Diego? I'm 18 and just looking to do it recreationally (mostly so I’m not just sitting around the house). What kind of sport is it exactly? Can someone give me a quick rundown of the basics?
Hey everyone, I’ve run into a bit of a situation, and honestly, I really hate going to the doctor. I’m pretty sure it’s just an ingrown toenail, so if anyone has some smart advice on how I can deal with this quickly and without too much pain—ideally without having to make an appointment—I’m all ears.🙂

Of everything you just said, that last point is the only one that actually hits home. It’s true—just look around. These little monsters are everywhere because everything is shaped by the current culture.
